PROGRAM GUIDELINES
*Patients’ portion of the treatment fee is due at the time of service.
*No refunds on premiums will be issued at anytime if participant does not to utilize dental plan.
This program is a discount plan, not a dental insurance plan. It cannot be used on the following:
- Any other dental office.
- Referrals to specialist.
- Services of injuries covered under workers’ comp.

✓ 20% OFF Additional Cleanings, Dental Sealants, Fillings, Core Buildups, Oral Surgery, Crowns, Veneers, Periodontics, Dentures, Partials, Implants, and Root Canals.

Child Preventative – $348 annual or $29/month
ADULT PLANS ANNUAL PREMIUM
Adult Preventative – $420 annual or $35/month
Adult Preventative Plus – $492 annual or $41/month
